Chronic Care Management is a service allowed for Medicare patients which enables patients to have greater access to our doctors and APRNs. A designated nurse reviews all patient charts in the program to determine past, current, and future needs of Medicare patients.

The ultimate goal of this program is to reduce medicare costs through timely treatment of acute and/or progressing chronic illness. Implementation of this program has reduced ER visits, hospital admission, and urgent care visits.
